Innova successfully completed both Phase I and Phase II of the HSMonitor, an EU Horizon 2020 PCP project focused on innovative ICT-enabled monitoring to enhance health status and optimize hypertension care in 2021. Innova participated as a supplier in Phase I of the CareMatrix project as a coordinator of CareInn consortium. Carematrix functional addresses early identification of individuals at risk, supporting interdisciplinary communication for individual care plans, providing decision support for proactive care, facilitating seamless transitions, and empowering self-management for those with multimorbidity. Additionally, in the InCareHeart PCP project, Innova took on the role of the leading supplier for the IncareNova solution—a comprehensive, intelligent Chronic Heart Failure (CHF) management system. This substantial contribution played a significant role in advancing integrated care for patients with Chronic Heart Failure within the framework of the InCareHeart PCP project.
